To assist parents/caregivers in creating a new treatment plan.
Beginnings are a great time to look back and evaluate your expectations, goals, and aspirations. We learn from our mistakes, we learn from certain decisions and choices that seemed right at the time, and we learn, too, that even with the best intentions sometimes those choices led us down a rocky road.
Use the worksheet on the following page, “Three-Stage Plan for an Amazing School Year,” to engage parents/caregivers in the child/teen treatment plan.
Ask the parent/caregiver the following questions:
1. What was the best thing that happened last school year?
_________________________________________
2. What was the worst thing that happened?
_________________________________________
3. If you could do something over again, how would you handle it?
_________________________________________
4. What advice would you give yourself for this new school year?
_________________________________________
5. What do you know now that you wish you knew a year ago?
_________________________________________
6. What are you concerns with your child today?
_________________________________________
7. What are your dream, goal, and vision for this school year for your child?
_________________________________________
Take one thing you want to improve or change, and break it down into three steps:
1. _________________________________________
2. _________________________________________
3. _________________________________________
I will complete these steps by _______________.
