This book exists because of Hellen Gelband, who asked me to coauthor the concept paper for what became the Geneva-based medical research nonprofit organization, Drugs for Neglected Diseases initiative. Hellen is now Associate Director for Policy at the Center for Disease Dynamics, Economics & Policy in Washington and New Delhi.
Three economists and friends offered support and sage advice that have been invaluable in forming my ideas and in critiquing these pages as they took shape. All were members of a remarkable group of colleagues with me at the Congressional Joint Economic Committee. Daniel Bond moved on to establish the country risk analysis protocols at the Export Import Bank of the United States in Washington and later performed in similar capacities at Duff & Phelps Credit Rating Company and Ambac Assurance Corporation in New York. Dan now serves as a consultant to several public and private groups. James Klumpner went on to serve as chief economist for both the House and Senate Budget Committees and more recently has been a Visiting Lecturer at Princeton University, George Washington University, and at Georgetown University. David Podoff became Senator Daniel Patrick Moynihan’s Chief of Staff for the Senate Finance Committee and is now an Adjunct Professor of Public Policy at Georgetown University and at the University of North Carolina– Chapel Hill.
Many others—some of whom will recognize their ideas in the text—were critical to the evolution of my thoughts.
