Cap is white to light brown or yellowish with pale wart patches, looking like the hide of a panther. It is convex, becoming flat to depressed in the center with age. Gills are white and free or slightly attached. Stalk is tapered, white, with a membranous, pendant collar. One or more concentric ridges form just above the basal bulb. Spore print is white. It is found in deciduous or coniferous forests, most commonly in the Rocky Mountains or Pacific Northwest during the summer or fall. Poisonous.
