Data Entry
Data is made up of the raw facts, numbers, letters, or symbols that the computer processes into meaningful information. Data is also the information that is put into the computer. The type of data in a typical office database would be the name of the care provider, patient demographic information, insurance carriers, diagnosis codes, procedure codes, and transactions. Following is a more detailed list of the types of data.
- Provider: name, address, phone number, tax and medical identifier numbers, and other information about the physician and the practice
- Patients: each patient’s medical record number and personal information
- Insurance carriers: name, addresses, other data about each insurance carrier that the physician accepts
- Diagnosis codes: codes from ICD-10-CM (International Classification of Diseases, 10th Edition, Clinical Modification) specify the diagnostic information about the illness, injury, or condition for which the patient is under treatment
- Procedure codes: procedure codes from CPT (Current Procedural Terminology) specify the care and treatment provided in the office setting; the codes are used for billing and charges for care rendered in your practice office
- Transactions: information about the patient’s visit, diagnoses, procedures, and received and outstanding payments, recorded in the form of charges, payments, and adjustments
