I have been offered and have accepted an exceptional opportunity as a member of senior management with another company, in another industry.

I am very grateful that I have had the opportunity to work for Consolidated Intergalactic for the past seven years. I will always think of the organization as a first-class company.

I have greatly enjoyed my career here at CI and will treasure my friends and our accomplishments.

Recommending employee for promotion

From department head to Human Resources

I wholeheartedly recommend Jamison Kennedy for promotion to a supervisor’s position, even though we will miss her very much as a Customer Service agent in our department.

Ms. Kennedy has demonstrated total dedication to her job and has pursued every available avenue to improve her skills and training while in our employ. I am certain she will be a success as a supervisor in the Shipping Department.

Objecting to proposed promotion for employee

From department head to Human Resources

CONFIDENTIAL

At this time, I am registering an objection to the proposed promotion of Jamison Kennedy from Customer Service to a supervisory position in the Shipping Department.

At this time I feel that such a promotion would hurt two departments. Ms. Kennedy is a capable agent in Customer Service, completing her first year in our employ; although I believe she will eventually warrant a promotion and additional responsibilities, she has only just recently mastered her assignment.

I would recommend we consider a promotion for Ms. Kennedy in six months or one year after we have had the full benefit of her work here in our department and after she has completed available training courses on management skills.

Acceptance of promotion

From employee to department head, Human Resources, or Employment Office

I am happy to accept a new position as senior benefits analyst in Human Resources, effective as soon as is convenient.

I have reviewed the job description and salary offer, and I am very excited about the new challenges the position will offer. I will do my best to exceed your expectations.

Thank you again.

Acceptance of transfer

From employee to Human Resources or Employment Office

I am pleased to accept your offer of a new position as director of Customer Service in our Columbus office.

I have reviewed the job description and salary offer, and I am very excited about the new challenges the position will offer. I will do my best to exceed your expectations.

I would appreciate a short period of time to consider the best schedule for moving to Columbus; my wife will be leaving her job here in Lumbertown, and we also need to consider the school calendar for our children. I will call you to set up a meeting to discuss a proposed schedule within the next business week.

Thank you again.

Declining promotion

From employee to department head, Human Resources, or Employment Office

Thank you for the offer of a promotion to senior benefits analyst in Human Resources. I appreciate the show of confidence by my supervisors and the company.

However, I have decided that the new position takes me in a direction different from my career goals. Therefore, I respectfully decline the offer.

Thank you again.

Request for transfer from one department to another

From employee to Human Resources or Executive Office

At the suggestion of my supervisor, I am writing to request a transfer from the Accounts Receivable Department to the employee Benefits Office.

I note that the employee Benefits Office has posted an open position as a financial analyst on the Human Resources internal Web site. Although this transfer is lateral in terms of job responsibilities and pay grade, I am making this request to broaden my background in financial management here at Consolidated Intergalactic.

I look forward to the opportunity to interview for this job. Thank you.

Declining promotion with transfer

From employee to Human Resources or Employment Office

It is with regret that I must decline your offer of a transfer to Columbus and a new position as director of Customer Service there.

Although I appreciate the offer of a promotion and the confidence my supervisors have shown in my abilities, at this time family considerations would make it very difficult for me to leave Lumbertown. My wife is currently employed here, and our young children attend school.

I hope you will consider me for other opportunities with the company here at headquarters, and with your permission I will advise you when my family situation is such that I could accept a transfer out of the area.

Thank you.

Request to be considered for internal job opening

From employee to Human Resources or Executive Office

Regarding the position of senior travel planner posted on the Human Resources Web site, I would like to submit my name for consideration.

I have worked for Consolidated Intergalactic for five years, currently as an executive assistant to the vice president for employee benefits. I have performed many of the components of the job description and have an extensive background in budget management. In expectation of such a staff opening, I have recently completed a college-level course on travel planning at Lumbertown Community College.

I look forward to the opportunity to interview for this job. Thank you.

Request for clarification of vacation policy

From employee to Human Resources or Benefits Office

I am writing to seek clarification of the company vacation policy. According to my records I have seven days of vacation available to me as of today, and I am planning to request vacation time off for a period six weeks from now.

1. Can you please confirm the number of days in my account?

2. According to my reading of the employee manual, I am accruing vacation days at the rate of one day for each completed month of service. Is that correct?

3. I f I schedule my vacation for six weeks from now, which would be the third and fourth weeks of August, how many days will I have in my account? Will I accrue my August vacation day while I am out of the office on vacation at the end of the month, and can I use that day as part of my upcoming vacation?

Thanks for your assistance.

Request for clarification of holiday schedule

From employee to Human Resources or Benefits Office

I am writing to seek clarification of the company holiday schedule. I do not celebrate Christmas, and for the past few years I have volunteered to work the emergency desk on that day so that others can be with their families.

I will be making that offer to my supervisor once again. I would like to know if I could be compensated with two personal days instead of being paid double-time for working on the holiday.

Request for clarification of overtime policy

From employee to Human Resources or Benefits Office

I have been asked by my supervisor to work at least six hours of overtime during each of the last four weeks of the fiscal year, beginning June 15.

I am aware that the company has the right to require a reasonable amount of overtime from its employees, and I am very willing to help in any way that I can, but my religious beliefs do not permit me to perform labor on Friday nights and Saturdays. I would be happy to work late on other nights of the week, or on Sundays.

Can you please clarify the company policy on overtime, and assist me in working out an acceptable schedule with my manager?

Request for clarification of use of accrued vacation time

From employee to Human Resources or Benefits Office

At the end of this year I will have approximately nine days of unused vacation time to my credit.

Can you please advise me of the date by which I must use that accrued time? Will the time be automatically converted to a cash payment if I do not use it? And if so, when would I receive payment?

Request for clarification of vacation policy, half-day vacation

From employee to Human Resources or Benefits Office

I am writing to request to split one of my available vacation days into two half-days off in August. I would like to leave work at 1 P.M. on the first two Fridays of the month to get a head start on weekend trips to our summer home.

I have consulted with my supervisor, and he said he has no problem with my proposed schedule. Please advise.

Request for flextime

From employee to Human Resources or department head

I would like to request a change in my working hours, as permitted under Consolidated Intergalactic’s flextime program.

I would like to adjust my schedule to begin my day at 8 A.M., one hour earlier than ordinary business hours, and leave the workplace one hour early at 4 P.M. This will allow me to better coordinate my commuting schedule with available childcare for my daughter.

Attached is the required memo of approval from my supervisor.

Request for bereavement leave

From employee to Human Resources or Benefits Office

I am writing to request a three-day bereavement leave, effective immediately, to attend the funeral of a family member. Tammy Carpenter, my sister-in-law, passed away yesterday at Yawkey Memorial Hospital in Lumbertown, and services are planned for Friday of this week.

I have advised my supervisor of my need to assist my brother and family.

Thank you for your consideration.

Waiver of confidentiality for employment details

From employee to Payroll Office or Human Resources

I hereby grant permission to Consolidated Intergalactic to disclose details of my employment to the Bank of Lumbertown in connection with my application for a mortgage.

Specifically, I give permission to inform the BofL of my job title, date of hiring, current status of employment, and monthly or annual salary.

This permission is effective immediately, and shall expire thirty days from the date of this letter.

Thank you.
