Social phobia is an anxiety disorder that is highly comorbid with other anxiety disorders, depression, and substance use disorders, such that up to 90% of people who are diagnosed with social phobia in their lifetimes will be diagnosed with another mental health disorder. However, rates of comorbidity differ significantly across studies. The reasons for these differences are unclear, as some studies using nearly identical methodologies obtain vastly different rates of comorbidity. Over the past 20 years, investigations of comorbidity in social phobia have extended beyond the consideration of comorbidity with other anxiety disorders, depression, and substance use disorders. This research indicates that social phobia is also highly comorbid with eating disorders and body dysmorphic disorder. Although this research has found lower rates of comorbidity between social phobia and bipolar disorder and psychosis, rates of the clinical conditions are nevertheless higher in people with social phobia than in people without social phobia. Research is now beginning to examine rates of comorbidity between social phobia and other mental health disorders in non-Western samples and in people of various ethnicities and those who immigrated at different times in their lives. Future research must establish rates of comorbidity between social phobia and other mental health disorders on the basis of DSM-5 criteria.

Note: All values in columns are percentages of the people with social anxiety disorder who were diagnosed with the comorbid disorder specified. Values in parentheses are odds ratios. Abbreviations are as follows: GAD = generalized anxiety disorder; OCD = obsessive compulsive disorder; PD = panic disorder; AG = agoraphobia; PTSD = posttraumatic stress disorder; MDD = major depressive disorder; NOS = not otherwise specified; OR = odds ratio; ---- = disorder was not assessed in the study. Cells that combine alcohol abuse and dependence and drug abuse and dependence give percentages and odds ratios that combine the diagnoses. Two studies (i.e., Grant et al., 2005; Mohammadi et al. 2006) provided separate rates of bipolar I and bipolar II disorder; in these cases, the percentages were combined, and the odds ratios for bipolar I and bipolar II disorders, respectively, are provided in parentheses.
