In a small bowl, whisk together 2 teaspoons water with the mayonnaise, sour cream, ketchup, relish, onion, vinegar, sweetener, hot sauce, and a pinch of salt. Set the dressing aside.
In a medium skillet, heat the oil over medium-high heat. Add the onion and cook, stirring, until translucent, about 2 minutes. Add the ground beef and 1 teaspoon salt and cook, breaking the beef up with a wooden spoon, until browned, 6 to 7 minutes. Add the garlic and mix together. Remove the pan from the heat and set aside.
Divide the romaine, tomatoes, and cheese equally among four large bowls or plates. Spoon the beef mixture equally over the salads. Drizzle each bowl with 2 heaping tablespoons dressing. Garnish with pickle slices, if using. Serve immediately.
