There are three ways to make it happen.

Manual

She strokes his G-spot with a well-lubricated finger or two or a slender anal vibe.

Oral

Shower or bathe together first. He gets down on all fours on the bed—the same position she assumes for rear-entry intercourse. From behind, she licks his anus in broad strokes, then penetrates him with the tip of her tongue. (For protection against bacteria, use a sheet of plastic wrap.)
